Heavenly Badonkers Angel Bitches takes place in a medieval fantasy world where angels hunt down sinners for punishment. To be more precise, the story involves teams of so-called "angel bitches", who travel the world to find and screw the brains out of sinners. While sex with an angel is as pleasurable as one might expect, it also drains the human body, leaving them immobilized for days.

The female angels can’t do their work alone, though. As such, they hire men to help them apprehend criminals and to sexually punish the female sinners they might come across. The protagonist is one of such holy warriors, assisting various angel bitches in their hunt for sinners.

The game features both exploration and traditional RPG combat, with the protagonist being paired with female angels on his adventure. The plot revolves around finding and punishing sinners. The outcome of combat is always meant to reward the player: either the angel will have her way with the sinner or the sinner will rape her.
