Tits and Heartthrobs is a slice-of-life visual novel developed by Tuomo, who also worked Bride for the Princess, Climax Heroines, and Helga: Euphorium’s Song. The story follows Motoshige (whose name can be changed), a gamer geek who spends most of his time at home. One day, his little sister, Leiko, asks him when he's finally going to get a girlfriend. She suggests joining an after-school club, since she managed to find a boyfriend at her own club. Not wanting to be outdone by his younger sibling, Motoshige visits a few clubs, through which he meets three girls with giant breasts.

There are three girls you can date, with three corresponding erotic CGs. You can choose between either Akoto, the playful girl from the gaming club who enjoys first person shooters; Shika, the determined swimmer from the athletics club; and Maiya, an outgoing girl in the music club who yearns to be an idol. Each day, players can choose the club they want to go to and spend time with a girl there. Once a player gets close to her, they can invite her home and introduce her to Leiko. Afterwards, the two of you can make your relationship official and have sex.

Plot
