The Fiendish Tragedy of an Imprisoned and Impregnated Girl is a horror RPG set within a dungeon controlled by bandits. The game’s heroine, a half-human, half-oni girl, was caught by brigands while searching for a cure for her ill mother. The men dragged her into a cell and began using her body for their own selfish pleasure, with no regard for the girl's own well-being.

The game is about Tana’s life as she's enslaved to the brigands. Players control the heroine’s behavior throughout her brief moments of respite and during the abuse, deciding whether to service the men according to their wishes or oppose them, drawing their ire. Tana will be treated differently depending on her behavior, leading to various rape scenes.

There are 14 CGs and 60 images total when including their variants. Two of these images come from Onichiku-ya!’s previous game, Fiendish Violation Quest, of which The Fiendish Tragedy of an Imprisoned and Impregnated Girl is a spin-off, originally developed as an extra to be packaged with that original game.
